Led by our partners at Got Transition, the National Center is launching a series of virtual meetings for Title V CYSHCN programs titled, “Pediatric to Adult Health Care Transition Strategy Spotlights”. The series will entail three one-hour interactive sessions designed to offer real-time technical assistance and support to Title V CYSHCN programs on advancing pediatric-to-adult health care transition (HCT).
